Mazel Tov! Registering with Orthodox Connection means you are making a very positive step toward changing your life. Take your time to fill out the Confidential Questionnaire. We need to know exactly what your expectations are and we want you to enjoy the process of sharing them with us.

Before you fill out the questionnaire, please note the following:

1. If you want to give us more information after you complete the form - we will welcome it. Send it by e-mail, by fax or by calling us directly.

2. Each client pays a processing fee of $180.00 (US) and a further $900.00 on engagement. We guarantee that we will work with you until you are engaged. Payment options may be found at the end of the questionnaire.

3. Within 24 hours of Orthodox Connection receiving your questionnaire, we will call to schedule an interview (to be conducted in person or by telephone).

4. In addition to the information you provide on the questionnaire, we will require a current photograph. Photographs may be sent by mail. Scanned images may be sent by e-mail. You may be assured of absolute discretion.
